According to a study published in JAMA Dermatology, less than 60% of US patients with chronic inflammatory disease (CISD) receive the requisite testing before beginning systemic immunomodulatory treatment.
Doctors recommend laboratory resting for the presence of tuberculosis, hepatitis, and other conditions before starting most systemic immunomodulatory agents in patients with chronic inflammatory skin diseases. However, the current testing patterns in the US are unclear.
Dr Maria Schneeweiss and colleagues at a hospital in Boston wanted to determine how often pretreatment testing is recommended for patients with CISD, such as psoriasis, hidradenitis suppurative, or atopic dermatitis.
The researchers examined over 122000 adults with CISDs who started using an immunomodulatory agent like methotrexate, interleukin, tumor necrosis factor, dupilumab, and apremilast. Data was obtained from the US commercial insurance claims databases from December 31, 2002, to December 31, 2020.
Dr. Schneeweiss wanted to know whether the study participants had been given a lab test within 6 months before starting treatment and up to 2 years after treatment.
The researchers observed that less than 60% received the recommended pretreatment testing. They also noted that complete blood cell count was the most common test performed in 41% to 69% of patients before initiating treatment. Between 11% and 59% of patients had tuberculosis screening within 6 months before treatment, and 3% had updated tests 1 year later.
In addition, the team observed that between 13% and 41% had hepatitis testing before treatment. They also noted low pretreatment levels for patients placed on apremilast, adding that the rates were comparable to those of dupilumab.
“The observed variations underscore the need for future research to elucidate the rationale behind testing decisions and to optimize testing practices for individuals initiating these therapies, taking into consideration the specific recommendations for each agent and the patient’s overall clinical profile and history,” they concluded.
