Race-associated tumor differences in prostate cancer may affect care among White and Black men, such as early diagnostic testing and targeted therapeutics, according to a recent study presented at the 25th Annual Meeting of the Society of Urologic Oncology.
Over 1,000 men (n=522 White; n=557 Black) with ERG-positive prostate cancer were analyzed for differential expression of prostate cancer-associated genes and specific biologic pathways implicated in Black men with the disease.
“Expression of the ERG gene is an early molecular alteration in one-half of prostate cancers. Additionally, ERG+ is found to be two-fold more prevalent in White men compared with African American men,” the researchers said.
The researchers used the GSE169038 dataset to examine Decipher whole-transcriptome expression profiles (22,236 transcripts) in men with prostate cancer who had a radical prostatectomy. Tumors were identified as either ERG-positive or ERG-negative.
Most of the ERG-positive tumors were found in White patients (40%) compared to Black patients (23%). Furthermore, the study found more than 5,800 genes to be differentially expressed in ERG-positive relative to ERG-negative tumors. In White patients, 1,987 (34%) genes were differentially expressed. More than 1,300 (23%) genes were differentially expressed in only Black patients.
In addition, the study findings showed race-specific disruptions to the E2F pathway in only Black patients and with p53 in White patients. In both sets of patients, ERG upregulation was associated with dysregulation of the androgen receptor pathway.
“As molecular testing continues to emerge as a standard component of cancer detection and treatment selection, a clear understanding of race-associated tumor differences will be necessary to ensure equitable care across racially diverse settings,” the researchers said.
