Individuals with uncontrolled severe asthma have greater impairment to health status and lower productivity than those with controlled severe asthma, according to a study published in the Journal of Asthma and Allergy.
Dr. Bo Ding and colleagues at AstraZeneca conducted a study to assess the symptom burden in people with uncontrolled severe asthma and controlled severe asthma.
The researchers used real-world data from the novel observational longitudinal study (NOVELTY), which included patient data from health centers in 19 countries across North America, South America, Asia, Australia, and Europe.
Participants in the NOVELTY study were 18 or 12 years and older who had been diagnosed with asthma, chronic obstructive pulmonary disease (COPD), or both.
The researchers defined uncontrolled severe asthma as having Asthma Control Test (ACT) scores less than 20 and/or physician-reported severe exacerbations from baseline evaluation in the previous year. On the other hand, uncontrolled severe asthma was defined as having an ACT score of 20 or greater and no physician-reported exacerbations in the previous year.
The researchers also included patient questionnaires to assess the participant’s health status. These questionnaires included the Respiratory Symptoms Questionnaire (RSQ), which is a measure of the severity of asthma symptoms (The higher the RSQ score, the more severe the symptoms); St. George’s Respiratory Questionnaire (SGRQ), which is a measure of how asthma affects a person’s quality of life (higher score indicates worse quality of life); and the EuroQoL 5 Dimensions 5 Levels Health Questionnaire (Eq-5D-5L) index value, which measures productivity (lower score indicates poor function).
The researchers identified 1078 patients with uncontrolled severe asthma and 315 with controlled severe asthma. The researchers found that patients with uncontrolled severe asthma had a higher symptom burden (RSQ score of 7.7 vs 2.5), worse health status (SGRQ score of 47.5 vs 22.4), and lower productivity (EQ-5D-5L index value of 0.68 vs 0.90) than those with controlled severe asthma.
Furthermore, the researchers found that people with uncontrolled asthma were almost 3 times more likely to miss school or work due to their asthma than those with controlled asthma (29.3% vs 10.5%).
The authors acknowledged that their study had some limitations, such as potential patient selection bias, variation in asthma and COPD diagnosis, and missing data. However, the researchers believe their study provides a better picture of the symptom burden, health status, and productivity loss among patients with uncontrolled and controlled severe asthma.
“These results support recommendations for interventions to achieve good asthma control in more patients with severe asthma, which would reduce its symptom burden and its substantial impact on patient health status and productivity,” wrote the researchers.
