<p>A study presented at the American Society of Clinical Oncology 2025 Annual Meeting investigated trends and disparities in colorectal cancer (CRC) mortality in the United States since 2018.<p>Pranav Chalasani of Wayne State University and colleagues utilized the CDC WONDER database to identify adults aged 25 and older with CRC from 2018-2023. The study team calculated crude mortality rates (CMRs) and age-adjusted mortality rates (AAMRs) per 100,000. They also stratified mortality data by age, gender, geographic region, and urbanization level.</p><p>Over the 5-year period, there were 313,744 CRC-related deaths. The AAMR consistently declined from 12.92 to 12.44 between 2018 and 2023.</p><h2><strong>Age and Geography</strong></h2><p>The highest CMR was in patients aged 85 years and older (156.11 per 100,00).</p><p>The Midwest and South had higher AAMRs (13.33 and 13.54) compared with the West and Northeast (11.90 and 11.54). The Northeast experienced a significant decline in AAMR, with an annual percentage change (APC) of -1.90 (95% CI, -3.24 to -0.58; <em>P</em>=0.01). Around one-quarter of deaths were from individuals in large central metro areas.</p><h2><strong>Gender and Race</strong></h2><p>Men had a higher CMR than women (17.23 vs 14.42). CMRs did not significantly change year-over-year during the study period for either sex.</p><p>Although Black individuals had a lower CMR than White individuals (15.7 vs 17.01), Black individuals had the highest AAMR of all races identified (16.18). However, Black individuals experienced a statistically significant annual decrease in AAMR (APC, -1.63; <em>P</em>=0.01).</p><p>&ldquo;These findings reveal critical gaps in CRC prevention and care, disproportionately affecting young adults, males, and minorities,&rdquo; the authors concluded. They called for public health initiatives to expand CRC screening and address regional differences in CRC care access and mortality outcomes.</p></p>
